Applying for a replacement of a lost white card

TAFE SA can only re-issue white cards that have been obtained through studies at TAFE SA.

As per SAFEWORK SA advice if you have not kept up your competencies by continuing to work in the construction industry in the past two years, your white card is not considered to be valid and you will need to enrol and complete the unit CPCWHS1001 Prepare to work safely in the construction industry.  You will not be able to apply for a re-issued card.

Replacement white cards will only be issued for unit of competency completed from 1 July 2011 onwards with a photograph present.

To receive a re-issued card, you will be asked to provide verification of identification, evidence of your original studies and confirm that you have been working in the construction industry over the last 2 years.

A separate application form and fee of $30 is required for each white card replacement requested.
