Parchment, Statement of Attainment including Competency Card and Extract Reissue

Applications must be supported by the following documentation:

  • Replacement: A statutory declaration (stating what has happened to the original) and photographic ID
  • Reissue: A statutory declaration (stating what has happened to the original) OR the original parchment, and photographic ID
  • Letter of Extract: Photographic ID
  • Change of Details form to be completed if you have changed your name

Note: Replacement and Reissue only used for qualifications which are nationally accredited. Statement of Attainments will only be issued for units of competency completed from 2005 onwards. A separate application form and fee of $50 is required for each Parchment and Statement of Attainment. Payment to be made within 7 Days of Billing. If you have not received your replacement/reissue within 30 days from payment please contact TAFESA on 1800 882 661.
