Note: Replacement and Reissue only used for qualifications which are nationally accredited. Statement of Attainments will only be issued for units of competency completed from 2005 onwards. A separate application form and fee of $50 is required for each Parchment and Statement of Attainment. Payment to be made within 7 Days of Billing. If you have not received your replacement/reissue within 30 days from payment please contact TAFESA on 1800 882 661.
Documentation MUST be presented to establish the applicant’s identity as both the person named on the original parchment/Statement of Attainment and the person to be named on requested documentation.
