Request for Withdrawal/Deferral/Release (pdf)
This form is to be completed by students who wish to apply for Withdrawal, Release or Deferral from their course at TAFE SA.
If a refund is required a Request for Refund of International Student Tuition Fees form will need to be completed.
Change of Course - Continuing Student (pdf)
This form is to be used by current TAFE SA international students wishing to change their course of study. Please note there is an Administration Charge for the 1st and any subsequent requests to change a course.
Change of Course - New Student (pdf)
This form is to be used by new TAFE SA international applicants wishing to change their course of study and requiring a new Letter of Offer and/or COE.
Extension to COE form (pdf)
This form is to be used each time a student requires an extension to their COE (and Student Visa). Email requests for COE/Visa extensions will no longer be accepted.
Request for Leave (pdf)
This form is to be completed by students who wish to apply for leave from their course at TAFE SA for two weeks or less.
Request for Refund (pdf)
This form is to be used by students who are eligible for applying for tuition fee refund.
Form Request for Extension or Payment Plan (pdf)
This form is to be used by students who can demonstrate genuine financial hardship may seek approval for an extension to the due date for payment of international student tuition fees or to pay tuition fees by installments.

Accommodation and Welfare Arrangement Form (pdf) (90 KB)
This form is for parents or guardians to confirm the choice of accommodation and welfare for students who are under 18 years of age.
Excursion/overnight stay form (pdf) (85 KB)
This form is to be used by International Students under the age of 18 years, for whom TAFE SA has accepted responsibility for accommodation, support and welfare, in order to apply for permission to undertake any excursion or overnight stay at an address other than the approved TAFE SA homestay address.
Request for holiday form (pdf) (88 KB)
This form is to be used by International Students under the age of 18 years, for whom TAFE SA has accepted responsibility for accommodation, support and welfare, in order to apply for permission to leave Australia or go on holiday overseas. Students must discuss these arrangements with the International Student Support Consultant at least four weeks before tentative travel date to ensure that they are granted approval before departure.
